Blepharoplasty (eye lift surgery) can make the eyes look more open and refreshed, but how long will that change last? Upper and lower blepharoplasty can both provide long-lasting results, although the treated areas continue to age in different ways. Here is what you can realistically expect in the years after surgery.
How Long Does Blepharoplasty Last?
Blepharoplasty is performed to address age-related changes around the eyes, such as excess upper eyelid skin, heaviness, or under-eye bags. An upper bleph removes excess skin and, when needed, addresses fat that contributes to a heavy or tired appearance.
The results are long-lasting because the tissue removed during surgery does not grow back. However, blepharoplasty does not stop the aging process. The skin and tissues around the eyes will continue to change over time, so new laxity or heaviness may gradually develop as you get older.
How Aging Affects Blepharoplasty Results
Blepharoplasty creates lasting changes, but the appearance of the eyelids will continue to evolve as you age. Several factors can influence how quickly new changes develop around the eyes.
- Skin elasticity and genetics: Natural differences in skin quality and elasticity affect how the eyelids age and when new laxity may develop.
- Brow position: The brow can gradually descend with age, creating heaviness above the eyes that may look similar to excess upper eyelid skin.
- Sun exposure: UV exposure contributes to the breakdown of collagen and elastin, which can accelerate visible skin aging around the eyes.
- Smoking: Smoking contributes to premature skin aging and can affect the quality and elasticity of the skin.
- Changes in facial volume: Aging and significant weight changes can affect the cheeks and surrounding tissues, changing the overall appearance of the eye area.
Can Blepharoplasty Be Repeated Years Later?
Blepharoplasty can be repeated years after the original surgery when new concerns develop and another procedure can be performed safely. However, having had an upper bleph or lower bleph in the past does not necessarily mean the same procedure will be appropriate again.
For patients who have had a previous upper or lower bleph, Dr. Brent Boyse evaluates the amount of remaining skin, eyelid position and function, brow position, and changes that have occurred since the original procedure. Removing too much skin can interfere with normal eyelid closure, so preserving eyelid function is an important part of planning revision surgery.
The goal is to determine what is causing the current concern rather than assuming another blepharoplasty is needed. A careful evaluation can determine whether the changes are appropriate for a repeat procedure and what approach may be best based on the current anatomy.

FAQs
Can upper bleph last 20 years?
Yes, the results of an upper bleph can still be visible 20 years after surgery. The procedure creates lasting changes, but the eyelids and surrounding tissues continue to age, so the eye area will not look exactly the same as it did immediately after surgery.
Does lower bleph last forever?
Lower bleph often produces very long-lasting results and rarely needs to be repeated. Aging can still cause thinner skin, fine lines, cheek descent, or volume loss around the treated area.
Does blepharoplasty make the eyes age faster?
Blepharoplasty does not accelerate normal aging. The eyelids and surrounding face continue to age after surgery, which can make later changes more noticeable when compared with the earlier result.
